An insert is one possible part of care

Inserts may be part of a clinician-guided or over-the-counter support strategy, but they must fit the shoe and foot. The 2023 heel-pain clinical practice guideline advises against using either prefabricated or custom orthoses as an isolated treatment for short-term pain relief. It allows their use alongside other treatments.

This distinction matters when an advertisement implies that an insert alone fixes plantar fasciitis. Ask what problem the device is intended to address and how its effect will be reviewed. A high price or a custom label does not remove the need for that explanation.

Assess the foot, insert, and shoe together

Bring the shoes you actually use to an orthotic appointment. A device that fits a roomy trainer may not work inside a shallow work shoe. The question is not simply whether the insert physically enters the shoe, but whether your foot can still sit comfortably and remain held at the heel.

If buying a prefabricated insert, read its intended use and fitting instructions before cutting or wearing it outdoors. Check that its shape does not create a concentrated pressure point. A pronounced sensation beneath the arch is not, by itself, evidence that the device is providing a useful correction.

Know what to do when it feels wrong

Follow the supplied wearing schedule instead of immediately using a new device for your longest day. Instructions may differ according to the orthosis and why it was prescribed. Keep a note of the location and timing of any discomfort so that a review can be specific.

If the insert causes significant pain, skin problems, or persistent difficulty, seek advice from the supplying service rather than modifying a prescribed device yourself. Review may involve its fit, the host shoe, or the broader care plan; repeatedly adding more padding can obscure the original problem.
